CLSA Training Allocation Funds
FY 2009/10

To use your MVLS training allocation funds to pay for workshop related expenses, complete and send us the following two forms: 

Workshop Registration Notification form [pdf]
Workshop Reimbursement form [pdf]

The Workshop Registration Notification Form should be sent to notify us that you have registered an employee(s) for a workshop.

The Workshop  Reimbursement Form should be sent to us when you would like reimbursement for the workshop expense.  The library director's signature authorizing the reimbursement must be on the form in order for MVLS to release the  training allocation funds for your library.
